If you wish to receive credit for participating in the seminar, please submit an application for credit.(Kanazawa University course students Only)

Seminar Participation Report

If you are enrolled in a Kanazawa University course, please send a participation report to the Kanazawa University Next Generation Hokushin Cancer Pro Office after attending a seminar hosted or co-hosted by the Next Generation Hokushin Cancer Pro.

Comprehensive Certificate Application Form

If you have completed a regular graduate course at Kanazawa University and have fulfilled the requirements for completion, you are required to send an application to the Kanazawa University Next Generation Hokushin Cancer Pro Office. Application forms will be sent to those who have fulfilled the requirements for completion, so please let us know.

NPO Certificate Qualification Application Form

If you have completed the Next Generation Hokushin Cancer Pro Course and wish to receive a certificate, please send the form to the office of the university in charge of each course you are affiliated with.
